If you need to establish care with a primary care provider and are seeking one, SSM/SLUCare offers several Tier One providers that can meet your needs. Find these providers and their information through the following resources:

SLUCare Website: Check the SLUCare website for a directory of providers affiliated with Saint Louis University, including their contact information and areas of expertise.

Saint Louis University 2025 Open Enrollment

Open enrollment is your opportunity to evaluate your existing Saint Louis University benefits package and consider new options for coverage in the upcoming year. SLU's open enrollment for the 2025 calendar year is Oct. 30 through Nov. 15, 2024. Open enrollment is submitted and approved in Workday.

Wellness Discount

All benefits-eligible employees will need to complete an annual wellness exam with your primary care provider anytime between Jan. 1 and Dec. 31, 2024, OR visit a Quest location for a biometric screening. A primary care provider (PCP) can be a family medicine doctor, internist, obstetrician/gynecologist, nurse practitioner, or physician assistant within your primary care provider's office.

Their comprehensive approach helps to ensure continuity of care and a more holistic view of your health. The monthly premium discount will increase to $70 for employee-only coverage and an additional $35 for employees whose coverage includes their spouse.

Directions for Wellness Discount

If you have participated in a wellness exam with your primary care provider between Jan. 1- Dec. 31, 2024, and are currently on SLU's Medical Plan, United Healthcare will send us a report of completion. You will receive verification from Employeewellbeing@slu.edu that you have met the requirements for the wellness discount.
If you want to do a biometric screening with Quest at a Quest Lab, schedule a screening at https://my.questforhealth.com. To register, use the registration key (SLU2024) and Unique ID (your SLU employee ID).
Your spouse/domestic partner will need to set up their account using your Employee ID + S. For scheduling questions, contact the Quest Service Center at 1-855-623-9355

Note: if you are setting up a new account you will need to make an appointment using the registration key (SLU2024)

All requirements for the calendar year 2025 must be completed by Dec. 31, 2024.
